That’s why we’re proud to announce our investment in Pyx Health, an engagement company focused on healthcare’s most vulnerable, hard-to-reach populations. With the support of a $47.5 million investment from S2G, Pyx Health has acquired FarmboxRx, a leading healthcare food company that pioneered fresh fruits and vegetables as a covered health intervention. Together, they are creating a unified, full-service platform to address some of the most pressing and preventable barriers to care.
The Hidden Costs of Unmet Needs
Food insecurity is a significant driver of poor health outcomes — and among Pyx Health’s member population, it ranks as the most frequently cited social barrier. When individuals lack reliable access to nutritious food, they’re less able to manage chronic conditions and follow treatment plans. In 2023, more than 18 million U.S. households experienced food insecurity, driving an estimated $52 billion annually in preventable healthcare costs linked to conditions like diabetes, hypertension, and obesity.
For Medicaid and Medicare populations, the picture is even more severe. Facing disproportionately high rates of both food insecurity and social isolation, these individuals frequently experience hospital readmissions and worsening chronic disease. Health plans, especially those serving Dual-Eligible and Exchange patients, are searching for scalable, cost-effective strategies to engage members before these issues escalate.

Rebuilding Trust Where It Matters Most
Co-founded in 2017 by Cindy and Anne Jordan, Pyx Health was built to tackle the invisible epidemic of loneliness in healthcare’s most marginalized populations. Today, Pyx Health supports over 6 million Medicaid, Medicare Advantage, D‑SNP, and Exchange members, combining a tech-enabled platform with human-based peer support to address a wide spectrum of non-medical drivers of health (NMDOH), including housing, transportation, and mental health.
FarmboxRx complements Pyx Health’s approach by using customized food boxes as an effective engagement tool. Under the leadership of founder and CEO Ashley Tyrner-Dolce and COO and CCO Meghan David, FarmboxRx has delivered more than 2.7 million food boxes through partnerships with managed care organizations. These boxes are tailored to patients’ health conditions and serve as a tangible way to incentivize behavior, such as scheduling preventive visits or completing risk assessments.
Together, Pyx Health and FarmboxRx offer a platform that meets individuals where they are — starting with immediate needs like food and extending into long-term care coordination and behavior change. The integration allows FarmboxRx’s nutrition solutions to be embedded directly into care plans, acting as both a trust-builder and activation trigger.
